Our distinctive approach
Life 3 Case Management offers on-site, off-site, home, and virtual visits, ensuring flexibility and accessibility. Our team consists of highly qualified professionals including RN-BSN, LMSW, Certified ABA specialists, and counselors, all supported by a wealth of community referrals.
What Case Managers Do:
1. Assess Needs: Meet face to face to figure out what medical,housing,food and mental health support.
2. Find Local Resources: Connect you with nearby doctors, WIC, Snap or specialized care
3. Follow-Up: check in by phone or in person. Making sure your family needs are met.
